Welcome to the Launch Day/Second Stage 2005 Survey! This survey, conducted by Dr. Jennifer L. Hoffman of UC Berkeley and Jonathan Fowler of the University of Wisconsin, collects information about the experiences of physical science majors at UC Berkeley who attended the Launch Day and/or Second Stage orientations. We know it's been a few weeks since the orientations; if you need a reminder of what they were like, you can check out the photos pages before continuing.

In this survey, we will first ask some factual questions about your personal characteristics, how you heard about Launch Day/Second Stage, and which events you attended. Then we will ask for your opinions about various aspects of the orientations and how they relate to your experiences so far this semester. Some of the questions ask you to write in responses; please answer these as fully as possible. We estimate it will take you 10-15 minutes to answer all the questions. At any time you may choose not to finish the survey, or to quit and return to it later; your responses will not be saved until you click the "Submit" button.

In exchange for completing this survey, you will be eligible to win a $50 gift certificate to Amazon.com; at the end of the survey we ask for your name and email address so that we can enter you in the drawing. We also ask whether you would be willing to participate in future studies of physical science majors, for which you will be similarly compensated. We promise to keep the requests infrequent and the surveys as brief as possible.

PERMISSION AND PRIVACY NOTICE: The data we collect will be analyzed in aggregate (i.e., in groups, not individually). By filling in this survey, you agree to allow us to use your responses as part of the aggregate dataset. Neither your name nor your email address will ever be attached to the data or survey results in any way. We will use your email address only for survey-related purposes. If you request to be added to the Society for Women in the Physical Sciences email list or mentoring program, we will share your email address with SWPS only. We will not reveal it to any other parties or organizations.
